This is an Infection Control Assessment and Response tool (ICAR) that can be used to help nursing homes and other LTC facilities prepare for coronavirus disease 2019 (COVID-19). This tool may also contain content relevant for assisted living facilities. LTC facilities can take steps to develop a comprehensive COVID-19 response plan.

When you conduct an assessment at an LTC, this is the tool you will use. The ICAR tool is available on the CDC website, see Resource 2A in our tool kit. ICAR Self-Assessment and/or related materials must take the following actions by the dates provided below to be in compliance: Conduct and submit a self-assessment for each of the provider’s facilities using the COVID-19 Long-Term Care ICAR Self - Everyone who visits a long-term care facility will be screened for symptoms of COVID-19 and have their temperature taken before being allowed to enter the facility.
